​Who should use the INZ 1015 form?

The INZ 1015 form is for applicants seeking a work visa based on employment, qualifications gained in New Zealand (Post-Study Work Visa), or humanitarian grounds (e.g., Victim of Family Violence).

​Who should not use the INZ 1015 form?

​Do not use this form if you are applying for a Partnership Visa (INZ 1198), Working Holiday Visa, Recognised Seasonal Employer Visa (INZ 1142), or China Skilled Work Visa (INZ 1158).

​What supporting documents do I need for the INZ 1015 form?

You must submit:
  • A completed and signed INZ 1015 form
  • Two passport-sized photos
  • A valid passport
  • A job offer and job description
  • Evidence of qualifications and work experience
  • Medical and police certificates (if required)
